Registration Forms
Please complete the registration forms ahead of time to reduce your wait time before your appointment. The preference is that you fill out the forms using our fillable PDFs, download and save them, and then email the completed forms to info@drrubinstein.com with the subject line: Registration and [Your Full Name].
If you do not have email access or cannot email the forms for any reason, you may print the forms and bring them to your appointment. However, emailing the completed forms in advance is preferred, as this will ensure you are all set up when you arrive and will help reduce time during your visit.
